At your appointment, your MD may or may not provide a Treatment Plan / Visit Summary afterwards. It is ultimately your responsibility, and choice, to take your own notes during your appointment, and
follow through with the recommendations.

These are specialty labs which can only be purchased with your physician's referral. There is an additional $25 kit fee for any specialty labs for additional testing, in addition to the cost of the lab itself, whether we recommend, have you order, or we order 1 kit vs 4 etc., a fee of $25 applies and
will be collected at the time of your appointment.
